Reasonable candidates share a few features: symptoms that match the deficiency pattern, labs that support low physiologic status, no active hormone-sensitive cancer, and a willingness to monitor and titrate slowly.

Medicine takes 15–20 years to metabolize new evidence into standard practice. The consensus statement on testosterone for women came out in 2019. We're roughly halfway through that adoption curve.
