Introduction By using the telehealth services provided by Meta Digital Developers DBA Direct Care DBA Direct Care AI (“Direct Care AI”) through its website, mobile apps, or other telehealth technologies (collectively, the “Direct Care AI Platform”), you are acknowledging and agreeing to this Telehealth Informed Consent. This consent supplements but does not replace the Terms of Use, Privacy Policy, or Notice of Privacy Practices governing your use of the platform.By creating an account, starting a consultation, or affirming your consent through the platform, you acknowledge that you have reviewed this consent form, understand the associated risks, and agree to receive healthcare services via telehealth technologies. A record of this consent is maintained in your file, and your continued use of the services indicates your ongoing agreement to this document and any updates.The healthcare services provided through the Direct Care AI Platform are facilitated by licensed providers within the Meta Digital Medical Network (“Providers”).

Prescriptions are not guaranteed and are at the discretion of the Provider. 8. Technical failures may occur, and you agree to hold harmless Meta Digital Developers, Direct Care AI, and affiliated entities for delays or disruptions in service. 9. Diagnostic tests provided by third-party labs may yield errors (e.g., false positives/negatives) that impact treatment. 10. Direct Care AI does not guarantee approval for treatment or prescriptions. Providers retain sole discretion over clinical decisions.
