Is Care Coaching the same as a dietitian or therapist?
No. Care Coaching is medically-centered support for your weight loss treatment, provided by Certified Medical Assistants. It is not registered dietitian services, not a prescriptive diet or eating restrictions, not mental health therapy, and not a substitute for other specialized professional services. Coaching covers GLP-1 side effect management, nutrition strategies for shrinking appetite, sleep, stress, emotional eating patterns, and social situations. If you need specialized care beyond coaching, your DirectCare AI clinician can refer you.
What if I want to pause or skip Care Coaching sessions?
Everything is at your discretion. Our concierge team reaches out to offer support and help maintain your appointment rhythm, but you can pause, reschedule, change cadence, or skip sessions at any time. Your Care Plan stays with you, when you're ready to resume, you pick up where you left off.
Will I lose muscle along with the fat?
You can lose lean mass on any aggressive calorie deficit, GLP-1 included — but the protocol is built to minimize it. Your Care Coach sets a protein floor (typically 0.7–1.0 g per pound of target bodyweight), pushes resistance training as a non-negotiable, and watches the rate of loss. If you're losing too fast, your clinician slows titration. Most male patients keep meaningful muscle through the program when they stick to the protein + lifting plan.
What about testosterone — will weight loss help?
For most men, yes. Excess body fat (especially visceral fat) converts testosterone to estrogen via aromatase, which is one reason heavier men often run lower T. Meaningful weight loss usually improves total and free testosterone, energy, libido, and recovery — though it isn't a substitute for TRT when one is clinically indicated. Who is not a candidate?
You should not start tirzepatide if you or a family member have a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), severe gastrointestinal disease (including gastroparesis), severe pancreatitis, severe kidney impairment, or if you are pregnant, planning pregnancy, or breastfeeding. The clinician will also evaluate BMI, medication interactions, eating-disorder history, and recent gallbladder disease. We turn down patients who aren't appropriate, at no charge.
What are the most common side effects?
Most patients experience some nausea, reflux, or constipation in the first 1 to 2 weeks of starting or stepping up a dose. Less common: vomiting, fatigue, injection-site reaction. Rare but serious: pancreatitis, gallbladder issues, allergic reaction. Your clinician will give you a side-effect protocol and an anti-nausea medication (Zofran) where indicated, and your Care Coach will run the side-effect-management module in your first sessions.
Do I need insurance? Can I use HSA / FSA?
No insurance is required, DirectCare AI is direct-pay. Most patients use HSA or FSA dollars; we can provide an itemized receipt with the appropriate codes. Cash, debit, and major credit cards accepted.
