What's the difference between RPM and CCM?
Remote Patient Monitoring (RPM) is the device side — connected glucose monitors, blood pressure cuffs, and scales that send daily readings to your care team. Chronic Care Management (CCM) is the human side — monthly 1-on-1 calls with a coach who reviews your numbers, your medications, and your overall plan. Most patients are enrolled in both.
Is this covered by insurance?
For this Remote Patient Monitoring and Chronic Care Management program, we accept most major insurance plans, including Medicare. After you fill out the form, our team verifies your benefits and walks you through any out-of-pocket costs before anything ships. Most Medicare patients pay $0 out of pocket.
Which conditions does the program cover?
Diabetes (Type 1 & Type 2), hypertension, obesity, congestive heart failure, COPD, asthma, kidney disease, and more. If you have two or more chronic conditions expected to last at least 12 months, you likely qualify.
Which CGM will I use — Dexcom or Freestyle Libre 3?
Your provider chooses the device that's the best fit for you. Both pair seamlessly with our care team and give continuous, fingerstick-free monitoring.
How does the care team actually help?
Health coaches review your trends, send check-ins, and flag anything concerning to your provider. You'll have someone to call when a number looks off — not a chatbot.
How fast does it ship?
After your confirmation call and benefits check, your CGM is sent to your home. Most patients are monitoring within a week.
Do I have to download an app?
The CGM uses its own free app for readings, but you don't need any technical skill. We walk you through setup step-by-step on your confirmation call.
